• Disability evaluations are scheduled through Disability Determination Services. If an appointment needs to be rescheduled, call DDS directly at 1-866-542-8113.

• At Tunstall Psychological Services, PLLC, we see only children under the age of 18 years old. This means that a parent or legal guardian MUST accompany the child. Legal guardians should prepare to have court documents showing their status as the legal caregiver of the child.

• The first part of the evaluation appointment is dedicated to obtaining information from the parent. Be prepared to provide detailed information about your child's history and referral concerns. 

​

• Next, your child will be evaluated by the evaluator using tests specifically requested by DDS. 

​

• Please be on time to ensure that the evaluation can take place in its entirety.

​

• LOCATION: There is a parking lot behind the building and families are welcome to enter from the back entrance.
